Pathway devices are driven by a Health Information Exchange, the integration engine for disparate electronic medical records. The physician portal provides real time updates to the health record. This will eliminate issues regarding patients receiving care from multiple sources in one day. Patients will have a tool to access their health information at any time and from any place. Credible educational information will always be at their fingertips.

For the 1 in 3 Americans who suffer with high blood pressure, taking an active role in managing their own health could mean healthier, longer lives. The mission is to create an interactive solution that would motivate and empower people to proactively manage their health. Through easy access and organization of their personal health information, relevant content, and social support, we hope to change the way patients think about their health, and make better-informed decisions for healthier lives.

Create an interactive solution which enables patients to better manage their cardiovascular health through a customizable, user-friendly interface. The solution should be holistic and help organize and provide easy access to personalized health information, relevant public data, and aggregated content (ie education, treatment, diet, exercise and social network).  The goal is to empower patients to easily and effectively manage their cardiovascular health.

Phase 1:
Entrants are required to submit a 5 minute video demonstrating the solution's capabilities and functionality, and a PowerPoint presentation (30 slides maximum) describing the solution. Entries should meet all the requirements detailed in the "Submission Criteria" section below. Teams will receive instructions on how to submit their entries after registering.

Phase 2:
4 phase 1 winners will be selected to further develop their submissions. Phase 2 will culminate in a live demo day session occurring in January 2012.

Areas for Consideration:

  • Health support systems (community/family engagement)
  • Making health education engaging to encourage learning
  • Incorporating risk profiling tools to promote positive behavior/lifestyle changes
  • Sharing information with appropriate Healthcare stakeholders
  • Leverage existing publically available data sources (ie, etc)
