With doctors visits limited to 15 minutes a patient, patient satisfaction is at an all time low. As interaction between practitioner and patient dwindles, rates of readmission rise, producing a costly and cyclical health care problem. This breakout aimed to demonstrate the dynamic ways in which companies and individuals are improving the inpatient experience and putting an end to the structural problem of patient dissatisfaction.
Lena Lattanzi guides and oversees the execution of product design and strategy and has played a large role in Vree Health’s flagship TransitionAdvantage service. At Health 2.0 Lena focused on Vree’s patient care coordination tool for post-discharge daily health checks, working with transition liaisons and other communication tools. She demonstrated TransitionAdvantage, which is designed to cultivate patient engagement and provide care coordination for patients post-discharge through an integrated platform including people, process, and technology.
